Personal Health Record (PHR)
What is PHR?
The WebMD personal health record (PHR) permits you to securely gather, store, manage, and share your own and your family’s health information --- when you want, where you want, and with whom you choose.
How can I make my PHR registration easier?
Registration is simple, just click on the highlighted text WebMD or visit www.WebMD.com/phr and register. Once registered, you will be prompted for all other responses. If you don’t know the information requested, you can always revisit the link and update your information. You are not required to enter information in every category in order to register your PHR. Additionally, you may be able to connect to WebMD through your employer or health plan.

Alabama Department of Senior Services provides a link to this service. ADSS does not collect any personal information from the client. All the information collected is maintained by WebMD.
